Privacy Notice

This is the privacy policy of Negros Occidental Comprehensive Health Program Information and Collection System. This document explains the policies for the collection, use, and disclosure of personal information all in accordance with Data Privacy Act of 2012 R.A. 10173.

The Data We Collect

NOCHP Information and Collection System collects members information starting from their filled-out Member Data Record Form verified by the NOCHP program coordinator and signed by the registering member. The Member Data Record requires the following information:

  • Name
  • Address
  • Gender
  • Date of Birth
  • Place of Birth
  • Civil Status
  • Contact Number
  • PhilHealth No.
  • PhilHealth Membership Category
  • Company/Employer Name
  • Average Monthly Income
  • Source of Income
  • Spouse Name
  • Spouse PhilHealth No.
  • Spouse Date of Birth
  • Spouse Occupation
  • Dependents Name
  • Dependents Date of Birth
  • Dependents Gender

Registering members will be required to provide the following: Birth Certificate, Marriage Contract (if applicable), Birth Certificate of Child/ren (applicable for dependents registration) and Senior Citizen ID (if applicable). The above-mentioned documents are needed for data verification conducted by our Program Coordinators.

How We Store and Use Data

NOCHP Information and Collection System holds personal data of our members, authorized users, hospitals and other medical information for the usage of the program. The program seeks to protect personal data and ensure that authorized users understand the rules governing and disclosure of the usage of the collected data. This ensures and requires users to conduct due diligence prior to providing a health care assistance to members and assure accuracy of data provided by the patients.

All the data collected are securely stored in our database owned and monitored by the Information and Communications Technology Division. Access to member's information is limited to NOCHPs authorized personnels who have a legitimate interest for the purpose of carrying out the priority healthcare assistance program created by the Provincial Government of Negros Occidental.

Our Disclosure of Data

NOCHP Information and Collection System does not share, sell or rent any members/users personal information with any sectors not affiliated to the Provincial Government of Negros Occidental or in our sole discretion, believe necessary or appropriate, in connection with an investigation of fraud, Intellectual Property Rights infringements, or other activity that is illegal or may expose us to legal liability. NOCHP may provide members/users information to provincial owned hospitals and departments if needed and shall be bound by privacy obligations.

The NOCHP Information and Collection System was developed with the collaboration of The Provincial Government of Negros Occidental Information and Communications Technology Division and the Negros Occidental Comprehensive Health Program Division.
